Ketonuria occurs with fasting, low-carbohydrate diets, prolonged vomiting, and, most importantly, diabetic ketoacidosis, a serious complication of uncontrolled diabetes. Why Urine Ketones matters
Ketone bodies appear when the body burns fat for energy instead of glucose. May indicate diabetic ketoacidosis (dangerous), starvation, prolonged fasting, low-carb diet, or alcoholic ketoacidosis.
Reading a Urine Ketones result
Results are reported as negative, small, moderate or large. A positive result means the body is burning fat for fuel because it lacks usable glucose (MedlinePlus). The key question is whether blood glucose is also high, because ketones with high glucose raise the possibility of diabetic ketoacidosis.
Common causes of a high result
- Diabetic ketoacidosis
- Fasting, starvation or an eating disorder
- A ketogenic or very low-carbohydrate diet
- Prolonged vomiting, including in early pregnancy
- Acute severe illness such as sepsis or burns, or a high fever
- SGLT2 inhibitor medicines, which can cause ketosis without high blood sugar
Caveat: The urine dipstick uses the nitroprusside reaction, which detects acetoacetate but not beta-hydroxybutyrate, the most abundant ketone body (Prisco 2006). In diabetic ketoacidosis the beta-hydroxybutyrate to acetoacetate ratio rises from about 1:1 to as high as 10:1, and with insulin treatment beta-hydroxybutyrate commonly falls long before acetoacetate (Laffel 1999). The strip can therefore understate the total ketone load and stay positive while blood beta-hydroxybutyrate is already falling. This site has no separate beta-hydroxybutyrate page.
